Memorandum from the Chief of Naval Operations, which includes reports on collisions involving two U.S. Navy ships in separate incidents in June and August 2017 which resulted in the deaths of 17 American sailors.  The Navy’s report on the events and actions that led to the collisions concludes that the accidents were preventable and caused by multiple failures by those on watch duty.  From the Defense Department
