Estimates that the federal subsidies, taxes, and penalties associated with health insurance coverage for people under age 65 will result in a net subsidy from the federal government of $685 billion in 2018.  This cost includes the cost of preferential tax treatment for work-related insurance coverage, the cost of Medicaid and Medicare for those under 65, and government payments for other kinds of health insurance such as the marketplaces established by the Affordable Care Act.  From the Congressional Budget Office
